Please submit your personal information and the names of your examination committee.  This form can be submitted as soon as your committee is set, but must be submitted at least five weeks before graduation and conferral of degrees.  Please proofread your title and use proper capitalization and punctuation. Do not use all caps for your title. The way it is entered on this form is how it will be printed on your transcript and in the ceremony program.
